What Is LSVT LOUD? Speech Therapy for Parkinson's Disease

Parkinson’s disease affects far more than movement. For many individuals living with Parkinson’s, changes in speech and voice are among the earliest and most impactful symptoms. Speech may become softer, less clear, and more monotone over time, making it increasingly difficult to communicate with family, friends, and coworkers. LSVT LOUD is an evidence-based speech therapy program specifically designed to address these communication challenges.

How Parkinson’s Affects Speech

Parkinson’s disease is caused by the progressive loss of dopamine-producing neurons in the brain. This affects the motor systems that control movement throughout the body --- including the muscles involved in speech production. Common speech and voice changes in Parkinson’s disease include:

  • Reduced vocal loudness: Speech becomes quieter, often described as a “soft voice” that others struggle to hear
  • Monotone speech: Reduced pitch variation makes speech sound flat and less expressive
  • Imprecise articulation: Consonants may become mumbled or unclear
  • Changes in speech rate: Speech may become unusually fast or exhibit hesitations and pauses
  • Breathy or hoarse voice quality: Changes in vocal fold function can affect voice quality

One of the most challenging aspects of these changes is that the person with Parkinson’s often does not realize their voice has become quieter. Their internal perception of their own loudness does not match reality, which means they may feel like they are shouting when they are actually speaking at a normal conversational level.

What Is LSVT LOUD?

LSVT LOUD (Lee Silverman Voice Treatment) is a standardized speech therapy protocol that has been extensively researched and validated over more than 25 years. It is the most well-studied speech therapy approach for Parkinson’s disease and has a strong evidence base supporting its effectiveness.

The program focuses on one primary target: increasing vocal loudness. By training the individual to “think loud” and produce a louder voice, LSVT LOUD also improves articulation, intonation, and overall speech intelligibility. The single focus on loudness simplifies the treatment for the patient while producing widespread benefits across multiple aspects of communication.

The Treatment Protocol

LSVT LOUD follows a specific, intensive treatment schedule:

  • Frequency: Four sessions per week
  • Duration: One hour per session
  • Length: Four consecutive weeks
  • Total treatment time: 16 sessions (16 hours)

This intensive schedule is a key component of the program’s effectiveness. The concentrated practice drives neuroplastic changes in the brain --- essentially retraining the motor systems involved in speech production. The intensity mirrors approaches used in physical rehabilitation, where concentrated, repetitive practice leads to the strongest outcomes.

Each session includes:

  • Maximum sustained phonation exercises (holding a loud “ahhh” for as long as possible)
  • High and low pitch exercises to improve vocal range
  • Functional phrase practice using real-life sentences and conversational speech
  • Hierarchical speech tasks that progress from single words to conversation

Daily Practice and Long-Term Maintenance

In addition to the four weekly sessions, clients are expected to complete daily home practice assignments. These exercises take approximately 10 to 15 minutes and reinforce the skills worked on during sessions.

After completing the four-week program, clients transition to a maintenance phase. Continued daily practice is essential for maintaining the gains achieved during treatment. Many clients also benefit from periodic “tune-up” sessions with their speech-language pathologist.

LSVT LOUD is appropriate for individuals at various stages of Parkinson’s disease. However, the best outcomes are typically seen when treatment begins early --- before speech changes have become severe.
